The Toole County Community Health Improvement Plan: 2017-2019
In 2016, organizations and volunteers collaborated to identify how to strategically address community health priorities to improve the health and well-being of Toole County. Community members, stakeholders, and partners used an assessment process to determine health priorities and to develop the plan, which aligns healthcare organizations and other partners in our community to improve the health of the residents. We all influence the community we live in. The residents of Toole County are invited not only to embrace their individual health and the health of their families, but also to work with us to make Toole County a healthier place to live, work, and play.
